Hazel Creek Dam is an earthen embankment dam located about 72 miles from Kirksville, Adair, Missouri.

The dam, which is on the Hazel Creek River, was primarily built for Recreation purposes, but also serves for Water Supply benefits.

The dam was designed by LARKIN & ASSOCIATES and commissioned in 1982 and is currently owned by CITY OF KIRKSVILLE.
